Apply for a Card

Anyone who lives, works, owns property, or is enrolled in a public or private school in Canton, MI is eligible to apply online for a Canton Public Library Card. Parent or guardian may apply for a library card for a minor under the age of 18 by completing the minor application portion of the form below.

Cards issued by online application expire in 90 days. To obtain a permanent physical card you must visit the library with proper ID as outlined in our Circulation Policy.

If you are under 18 years old, parent or guardian authorization is required to complete this form.
Parent or guardian is responsible for the use of this card and any related charges per the library’s policies and is legally responsible for all materials borrowed on this card. Library staff will not monitor the use of this card and the borrowing of materials and/or computer usage on this account. A completed application form will be available for parent/guardian to sign when you visit the library and verify your eligibility.

I can change or void this list at any time by presenting identification to the library and requesting that individuals be removed or added to the list. I understand that such change requests must be made in person.
